The lighting in this picture makes her skin look lighter, but there's another picture on that website and her skin is the same color as before. I'm also not sure if she actually had a nose job or not, because even in old pictures, she had a pretty slender nose. I think the blonde hair and makeup are what's making her look haggard though.
